School-Based Health Center

Community First Health Centers is excited to announce the opening of a School-Based Health Center inside Algonac JR/SR High School (5200 Taft Rd., Clay Township, MI 48001). This Health Center will be located right inside the school and is available to individuals between the ages of 5-21. Residency in Algonac or Clay Township is NOT required.

Care will be provided by Haleigh Groomes, a board-certified Family Nurse Practitioner, who has several years of nursing experience in urgent care and public health settings. The center will be open Tuesday through Thursday from 7:30 AM – 4:00 PM, with after-hours services available for established patients.

• Immunizations
• Screenings
• Health Education
• Prescriptions
• Basic lab work
• COVID-19, flu and strep testing
• Pregnancy & STI testing
• Support for chronic condition

How to Register Your Child

If you would like your child to receive care at the School-Based Health Center, you will need to complete the necessary forms. Those include consent forms, registration forms and insurance information. Once completed, forms can be returned to the School-Based Health Center at Algonac JR/SR High School. Students can stop by the health center to pick up the forms or you can call 810-777-9924.

Friday-Sunday: Closed

Most major insurance plans are accepted! There will be sliding fee scale options available for eligible patients.
