Community-Based Health Care Services
Community First Health Centers has sought to make a positive impact on Michigan’s St. Clair and Macomb Counties for over 40 years. We provide community-based health and human services that promote the physical, mental, and emotional health of all members in the community.
Since 1975, Community First Health Centers has provided quality medical, behavioral, and educational health and support services to the communities of St. Clair and Northern Macomb Counties, assisted by private and public funding sources. We are proud to bring all of your health care needs into one Patient-Centered Medical Home Practice by offering all of the following services: Primary Health Care, Affordable Care, Transportation Services, Homeless Health Care, Behavioral Health Care, Lab & X-Ray Services, and WIC Services. When needed, language translation (including sign language and methods for communication with the hard of hearing and deaf) are utilized during appointments and care staff interactions.

Coming Soon – Port Huron Teen Health Center
Community First Health Centers is excited to announce the upcoming opening of the Port Huron Teen Health Center, located on the Port Huron High School campus. Opening July 27, 2026, the Teen Health Center will provide convenient, high-quality primary care services for children and adolescents, including wellness exams, sick visits, sports physicals, immunizations, preventive care, and counseling services. We are proud to partner with the Port Huron Area School District to ensure students have access to the healthcare they need to stay healthy, succeed in school, and thrive. Additional information, including hours of operation and appointment scheduling, will be available soon.
The Port Huron Teen Health Center Community Advisory Council will meet on July 9, 2026, and January 20, 2027. Community members, parents, students, and school representatives are encouraged to participate. For more information about Community Advisory Council meetings, please call (586) 270-8055.
