Community First Health Centers is proud to accept most commercial insurance plans, ALL forms of Medicaid, and patients with no insurance. Please call your local Community First Health Centers office today to discuss your insurance plan or to enroll in our Sliding Fee Program!
Our Sliding Fee Program is for those who are uninsured or underinsured. We offer medical, dental, and behavioral health services at a reduced rate based on a case-by-case eligibility. Some lab services will be covered when ordered by our providers at our centers; all other lab tests are the responsibility of the patient. Discounts do not apply to charges imposed by hospitals, laboratories, or other physicians. As about enrolling and qualifications in this program when you call to schedule your appointment!
Effective September 2020, Community First Health Centers has updated its Sliding Fee Program scale for dental procedures. If this affects you, please contact our eligibility specialist or speak to the dental team before your next scheduled appointment for updated information.
If your insurance plan requires a co-payment, you will need to pay this at the time of your visit.
We will refer you to any specialist that is appropriate for our medical situation.
If your provider determines you require a referral to another provider, and your HMO/Managed care plan requires a documented referral, you will be referred to one of our Care Continuum Specialists to coordinate your referral. He/She will work with your insurance company to ensure that your visit with the specialist is covered before your appointment.
To initiate a new referral or to check on the status of a referral, please contact your local Community First Health Centers office.
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room that you are a patient of Community First Health Centers, and they will contact us.
As a Patient-Centered Medical Home, we want to ensure you're supported throughout the day, even when we are closed. If you have an urgent medical need and would like to speak with one of our primary care providers, call our after hours phone line. Your call will be returned by the Community First Health Centers on-call provider promptly.
