Mobile Medical Outreach Request Form

Our Mobile Medical Team is currently accepting requests for our mobile van to be on location at social service organizations throughout Macomb and St. Clair Counties. Our team is available to provide limited health screenings, help residents establish a primary care provider, enroll residents in Community First Health Centers Homeless Health Care Program. We also offer vaccination clinics.
